Sage Group Health Care Staffing is seeking Home Health LPN for a 13-week travel/ contract position in Lynn Haven, FL. Accepting both local and traveler candidates.

Pay Rate:
Range $1320 to $1400 Weekly Gross pay (Non-taxable travel/housing and M&I stipend available for qualified/eligible travelers)
Mileage Reimbursement: $0.49/mile for distance covered between patient home appointments
Unit: Home Health
Schedule: 40 hours / week guaranteed, 5 shifts per week, Mon-Fri
Position Requirements :- LPN experience: 2 years
- Required - Recent/Current Home Health Nursing experience: 1 year within the last three years
- Required - OASIS experience
- Required - Home Care Home Base (HCHB) charting experience
- Required - Must have valid Driver’s License and Personal Auto Insurance
- Must be Covid Vaccinated orprovide medical or religious exemption
- ASN or an equivalent – Required
- BSN or an equivalent – Highly preferred
- LPN License for State of Florida or Compact
-Required - BLS Certification from AHA or American Red Cross
- Required
- Provide and coordinate care under supervision for assigned patients in comfort of their homes
- Support patient recovery andability to stay at home - enabling them to spend quality time with their loved ones and doing the activities they enjoy
- Empower patients and families toreach healthcare goals by educating them on disease management
- Ensure patients’ status is assessed on an ongoing basis and pertinent information is gathered and documented.
- Notify physicians of significant changes in status
- Document and complete charting for all patient visits within 24 hours.
- Territories to cover:
Panama City and surrounding counties
