Pay range: $65,000.00/yr - $85,000.00/yr (provided by Lynn Community Health Center; actual pay will be based on skills and experience).

Summary

The Reintegration Grant Program Manager oversees the BSAS‑BLM grant work including community interaction, grant reporting, compiling data, direct patient support, and interdisciplinary care consultation with the goal of creating positive transitions for individuals released from incarceration into the community and reducing recidivism.

The program works directly with Middleton House of Corrections and three facilities, including the Essex County Pre‑Release and Re‑Entry Center, a security facility for men that includes a substance use treatment unit. Data reports from the Sheriff’s Department indicate that inmates released from the CASE Units have the highest recidivism rates.

Key responsibilities include day‑to‑day functions of patient recruitment, enrollment, and ongoing care; working with grant funders; coordinating with addiction teams for timely enrollment in MAT; providing guidance and support to enrolled patients; maintaining communication with corrections staff; expanding in‑person visits; and building relationships with community recovery programs.

Essential Duties & Responsibilities
  • Manage workflow for grant requirements (enrollment, engagement, community collaborations)
  • Supervise a 0.5 FTE reintegration Community Health Worker
  • Ensure collaboration with outpatient, inpatient multidisciplinary team, and house of corrections
  • Facilitate and lead community groups for program participants
  • Conduct intakes and provide case management
  • Develop written materials and workflows for patient education and engagement
Education & Experience
  • 5+ years of related experience delivering community‑based services
  • 5+ years of experience and/or training in substance abuse/behavioral health
  • Preference for individuals who have worked in an HOC setting
  • Experienced in group facilitation
  • 3+ years of experience as a community health worker or case manager
Qualifications
  • History of good attendance and a positive work attitude
  • Strong organizational and communication skills
  • Knowledge and understanding of organizational culture and the culture of Lynn
  • Ability to work independently and analytically to meet milestones
  • Effective time management
  • Superior communication, planning, and client‑service skills
  • Demonstrated integrity, dependability, sound judgment, team‑building, and resourcefulness to develop collaborative relationships
  • Ability to listen, respond, and problem‑solve efficiently to address concerns of LCHC
  • Strong computer skills (MS Office suite)
  • Demonstrated supervisory or management experience
  • Excellent written communication skills
