RN and LPN Home Health Nurse

Jethro Health System is seeking an experienced RN to provide home health care across the North Shore area:
Lynn, Peabody, Nahant, Revere, Winthrop, East Boston, Chelsea, Salem, Swampscott, Wakefield, Reading, Woburn, Saugus, Revere.

Positions available as per diem or part-time with potential for full time.

• Conduct thorough assessments of patients' health status and needs in their home environment.
• Develop and implement individualized care plans based on patient assessments and medical history.
• Provide education and support to patients and their families regarding health conditions, medications, and self-care techniques.
• Assist with discharge planning from hospitals or rehabilitation facilities, ensuring a smooth transition back to home care.
• Monitor patient progress and adjust care plans as necessary to meet evolving health needs.
• Collaborate with interdisciplinary teams, including physicians, social workers, and therapists, to coordinate comprehensive care.
• Document all patient interactions accurately in compliance with ICD-10 coding standards.
• Offer health coaching to empower patients in managing chronic conditions effectively.
• Provide specialized services such as esthetic laser treatment when applicable.
• Support family planning initiatives and educate patients on related health topics.

• Strong clinical skills with a focus on acute care and senior care practices.
• Excellent communication skills for effective patient education and collaboration with healthcare teams.
• Proficient in discharge planning processes and the ability to navigate complex healthcare systems.
• Knowledge of ICD-10 coding for accurate documentation of patient encounters.
• Experience working in a teaching hospital environment is preferred but not required.
• Ability to provide compassionate care while maintaining professional boundaries.
• Strong organizational skills to manage multiple patient cases effectively.
• Commitment to ongoing professional development and staying current with best practices in nursing care.

RN - Oasis Start of Care: $100 per visit. RN - Recertification: $60 per visit. RN - Oasis Resumption of Care: $80 per visit. RN - Skilled Nursing visit: $45 per visit. LPN - Skilled Nursing visit: $40 per visit. Hourly pay: $37.73 - $45.00 per hour.

Flexible schedule, referral program.

Day shift, evening shift, weekends as needed.

Home care: 1 year (Preferred).

RN License (Required). BLS Certification (Preferred).

In Person. North Shore area (Lynn, Peabody, Nahant, Revere, Winthrop, East Boston, Chelsea, Salem, Swampscott, Wakefield, Reading, Woburn, Saugus).

Full-time, Part-time, Per diem.
