No nights, weekends, or weekends
Hybrid schedule 2 days remote, 3 days in the field/office

We're hiring Registered Nurse Case Managers to join our Senior Care Options team. You'll support older adults in staying healthy, safe, and independent at home while enjoying a predictable schedule and hybrid flexibility.

What You'll Do:Complete in-home assessments and create care plans
Manage a panel of members to ensure coordinated, high-quality care
Work with providers and community partners to meet member goals
Educate members and families to promote wellness
What We’re Looking For:Active MA RN license (required)
Nursing experience (home care, case management, or community health a plus)
Strong communication and organization skills
Reliable transportation for local travel
