Job Summary

Under the supervision of the Wraparound Program Lead, the Wraparound Parent Partner works closely with the Wraparound child’s parents/caregiver to represent their best interests and participate as a member of the Child and Family Team. The Parent Partner provides in‑home support services as outlined in the Plan of Care. The Parent Partner completes all required paperwork in accordance with agency and county standards.

  • Shares the story of own journey in order to connect, empower, and give home to parents.
  • Adheres to a non‑judgmental, strength‑based model that builds on strengths and hope, while also being realistic and practical.

(engagement, teaming, assessment and understanding, long term view, planning, cultural competence, transitions, crisis management).

  • Work closely and maintain weekly visit with parents and caregivers to provide intensive and supportive services needed
  • Link and advocate for resources in the school setting and community to address the family’s needs identified in the family’s plan
  • Assist parents in identifying needs, barriers and problem solves to assist in the implementation of the family’s plan
  • Collaborate and team with other professionals to plan and coordinate immediate services to stabilize sudden crisis
  • Document all services provided following the Wraparound and DCFS standards (i.e. DMH billing, contact notes, SIR’s, Flex funds, CFT minutes, Authorization forms, etc.).
  • Meet for individual meetings with both the out‑of‑home caregiver and parent/current caregiver.
  • Participates in weekly Child and Family Team (CFT) meetings
  • Works collaboratively and cooperatively within a multidisciplinary team
  • Consult on regular basis with supervisor and team members about pertinent issues regarding the family’s participation and treatment plan
  • Assist parents/caregivers in keeping appointments, access and link parents/caregivers to court hearings, and other community or county programs.
  • Available 24/7, responsible for responding and supporting parents/caregivers during crisis calls
  • Sensitive and responsive to the cultural differences among the agency’s client population
  • Participation in all agency required trainings which includes but is not limited to Wraparound Basic Training, Trauma Informed Care and Elements of Wraparound
  • Actively participate in parent support groups/activities and quality improvement projects
  • Adheres to all applicable Federal and State regulations for confidentiality, HIPAA, mandated reporting of suspected child or elder abuse
  • Actively participate in quality improvement projects
