GGZ-Delfland is op zoek naar een Buurt GGZ’er in Schiedam. In deze rol ben je de ogen en oren in de wijken van Maassluis, Vlaardingen en Schiedam. Je hebt een signaalfunctie waarbij je vroegtijdig problemen en isolatie van inwoners kunt signaleren en oplossingen kunt bieden.
Je taken omvatten vroegsignalering, outreachend werken, en samenwerken met lokale partners. Dit is een functie met een belangrijke rol in de gemeenschap, met aandacht voor diverse problematiek zoals verslaving en psychische aandoeningen.
