Care Coordination RN

The Care Coordinator RN (CC RN) assesses transitional care needs, coordinates across the care continuum, and engages patients and families to ensure care goals are met.

• Conduct psychosocial and functional assessments, plan and manage discharge processes, facilitate interdisciplinary rounds, and address barriers to care and discharge.
• Collaborate effectively with physicians, care teams, and social services to promote timely, efficient patient transitions.
• Document assessments, care plans, and interactions accurately, and ensure compliance with organizational policies.
• Support professional development, serve as a mentor if appropriate, and perform other duties as assigned.

• Requires an Associate’s or Bachelor’s Nursing degree, RN licensure (single or multi-state), BLS certification, and at least 1 year of nursing experience in acute care.
• Must possess excellent communication, assessment, organizational, and problem-solving skills.
• Ability to work well under stress, handle change, and collaborate with multidisciplinary teams.
• Knowledge of regulations and community resources is essential.
