Role:The Care Coordinator RN (CC RN) assesses transitional care needs, coordinates across the care continuum, and engages patients and families to ensure care goals are met.
Responsibilities:Conduct psychosocial and functional assessments, plan and manage discharge processes, facilitate interdisciplinary rounds, and address barriers to care and discharge. Collaborate effectively with physicians, care teams, and social services to promote timely, efficient patient transitions. Document assessments, care plans, and interactions accurately, and ensure compliance with organizational policies. Support professional development, serve as a mentor if appropriate, and perform other duties as assigned.
Qualifications & Skills:Requires an Associate's or Bachelor's Nursing degree, RN licensure (single or multi-state), BLS certification, and at least 1 year of nursing experience in acute care. Must possess excellent communication, assessment, organizational, and problem-solving skills. Ability to work well under stress, handle change, and collaborate with multidisciplinary teams. Knowledge of regulations and community resources is essential.
