An exciting opportunity has arisen for an enthusiastic and forward‑thinking Band 6 Nurse to join the Transfer of Care Hub (Integrated Discharge Team) Fixed Term or Secondment until end of March 2027.

Responsibilities
  • Promote the Discharge to Assess model by facilitating safe and timely discharges for patients from Macclesfield District General Hospital, supporting the Acute Medical Ward and, as necessary, for patients in Community Pathway 2 beds transfer to Pathway 3.
  • Support the completion of assessments and continuing healthcare checklists within the pathway 2 beds.
  • Coordinate and facilitate the safe and effective discharge of patients from Macclesfield District General Hospital, supporting proactive reduction in delayed transfer of care.
  • Provide assessments for discharge pathways 1-3.
  • Work proactively within an integrated health and social care team to promote formal and informal channels of communication both within the team, ward staff, ICB and wider community as appropriate.
Qualifications
  • Passion for putting the patient at the centre and experience of working collaboratively with multi‑professional partners.
  • Graduate visa holder with a minimum of 6 months before expiry at the start of employment.
  • Must have an Enhanced DBS (Disclosure and Barring Service) check with barred lists; the cost is payable by the successful candidate.
  • If you have lived or worked abroad for 6 months or more in the last 5 years, you will need to obtain an overseas police clearance.
