Hospice Registered Nurse Case Manager

Job Description

We are seeking an experienced and compassionate Registered Nurse (RN) Case Manager to join our Hospice team. This role is responsible for managing a patient caseload of 12-15, coordinating interdisciplinary care, and ensuring comfort and dignity for patients and end-of-life.

• Manage and coordinate care for a caseload of hospice patients in Oakland County
• Perform comprehensive nursing assessments and develop individualized care plans
• Collaborate with physicians, social workers, aides, and chaplains
• Provide education and emotional support to patients and families
• Ensure compliance with Medicare hospice regulations and documentation standards
• Participate in education groups at facilities

• Active RN license in the State of Michigan
• Minimum 1 year of hospice or home health experience preferred
• Strong clinical assessment and critical thinking skills
• Excellent communication and compassion for end-of-life care
• Reliable transportation, valid driver’s license and car insurance
