• Coordinate with other social service providers to ensure comprehensive care for families
• Participate in team meetings and case reviews
• Assess family needs and develop tailored plans to address those needs
• Provide case management services including supervised visits, parent aide, behavioral aide, drug screens, crisis intervention, and advocacy
• Conduct regular home visits to monitor progress and address issues
• Offer parenting education and support to improve family dynamics
• Collaborate with professionals such as social workers, therapists, and teachers for comprehensive support
• Maintain detailed case notes and records of services and progress
• Provide transportation services as needed

• Bachelor’s Degree in human services disciplines such as Social Work, Psychology, Sociology, Counseling, Criminal Justice, or related fields, or High School Diploma with 5 years of relevant experience
