Care Manager: Discharge & Care Coordination

A major health system in Macon, Georgia is seeking a Care Manager to develop and implement discharge plans for patients, coordinate care with a multidisciplinary team, and utilize clinical judgment in decision-making. The ideal candidate should have at least three years of experience in acute care or home health and a Bachelor’s degree in Social Work. This role requires strong interpersonal skills and certification in Care/Case Management preferred.
