Overview
Travel Nurse Practitioner at Monogram Health. Monogram Health is seeking a skilled Nurse Practitioner to provide access to comprehensive care for underserved patients within the Monogram Health network. The role focuses on helping patients improve quality of life in the home and slow the progression of chronic conditions, including kidney disease, through coordinated home-based care.
Responsibilities- Deliver personalized and compassionate care to patients primarily with CKD through ESRD.
- Serve as a nationwide float provider in areas where local clinicians are unavailable or during growth periods, conducting in-home assessments and management visits in patients homes or via virtual encounters.
- Conduct patient health assessments, develop health improvement strategies, and promote healthy habits.
- Maintain accurate patient records, scheduling, and follow-up appointments across multiple markets.
- Coordinate care with primary care providers, specialists, and ancillary services.
- Complete all documentation in a timely manner and uphold quality of care standards.
- Travel to work states where employed, attending periodic trainings or meetings outside of that state.
- Deliver evidence-based, timely care to reduce avoidable hospitalizations and to maximize quality of life and patient satisfaction.
- Prescribe medications, order tests, and collaborate with Monogram physicians as appropriate.
- Document clinical information: chief complaint, diagnoses, medical history, review of systems, examinations, medications, allergies, assessment, and plan.
- Close gaps in care by educating patients about preventive monitoring and coordinating diagnostic testing with physician practices.
- Assist patients with enrolling in educational resources and participate in integrated care team meetings.
- Maintain knowledge of disease diagnosis and prevention; assess patient health status and implement appropriate care plans; conduct follow-ups and evaluations.
- Other duties as assigned.
- Active and unrestricted RN and NP licensure.
- Board certification for NP (ANCC/AANP).
- Current and unrestricted DEA certificate or eligibility.
- Ability to travel extensively by air and use rental vehicles; work autonomously without direct supervision.
- Access to transportation, valid driver s license, and car insurance.
- Proficiency with medical instruments and equipment used in practice.
- Experience with computer-based data management, medical records, and related information systems.
- Strong verbal and written communication with patients, families, and medical partners at various levels.
- Understanding of federal and state regulations including HIPAA and OSHA.
- Two or more years of direct patient care experience required.
