About the Role

River Vista Behavioral Health sits along the bluffs overlooking the San Joaquin River. This brand new, 128‑bed facility will provide high‑quality and advanced behavioral health services to residents and visitors in the Central Valley. The new hospital will employ more than 250 people, including clinicians, nurses, mental health technicians, support staff, and administration.

We are currently seeking a forward‑thinking and compassionate LVN/LPT to be part of our Nursing Team. The LVN/LPT administers nursing care to designated patients under the direction and supervision of a Registered Nurse and performs a variety of direct and indirect patient care duties.

Responsibilities
  • Assist in the delivery of patient care in an acute psychiatric setting for patients experiencing a wide range of psychiatric conditions.
  • Provide nursing care, within the scope of practice, to meet the unique needs of each patient.
  • Assist the Charge/Staff Nurse by collecting data related to the patient’s condition.
  • Report data findings related to patient’s condition to the Charge Nurse.
  • Assist the Registered Nurse in assessing for pain, utilizing a pain scale and documenting accordingly.
  • Assist the Charge/Staff Nurse to organize and deliver patient care, and to supervise staff.
  • Document patient activity and behavior in an accurate, timely and legible manner.
  • Recognize when a patient is in need of 1:1 intervention and interact therapeutically; inform Charge/Staff Nurse of significant developments and document in progress note.
  • Provide 1:1 intervention as directed and report significant developments to Charge/Staff Nurse.
  • Assist the Charge/Staff Nurse in assessing the patient’s response to interventions; communicate findings to other team members.
  • Respond to patient in a manner that takes into account diagnosis, strengths, weaknesses, and current level of functioning.
  • Transcribe and administer medications and treatments accurately; document correctly and legibly on medication record.
  • Initiate infection control form and add to treatment plan when medications ordered indicate a medical problem.
  • Demonstrate responsibility for proper safekeeping, monitoring, and accurate recording of all controlled medications kept on the unit.
  • Follow safety procedures and secure contraband.
  • Keep Charge/Staff Nurse informed of events that may change staffing needs such as admissions, discharges, transfers, and patients in crisis.
  • Provide education to patients based on learning needs; document education about medications, disease management and coping mechanisms.
  • Notify Charge/Staff Nurse of patients with potential risk of injury due to falls or self‑destructive or assaultive behavior; implement fall precautions and communicate risk to others.
  • Demonstrate effective management of assaultive behavior, use of seclusion and/or quiet time.
  • Demonstrate use of most facilitative measures prior to seclusion and restraint: 1:1, diversional activities, groups, PRN medications, peer support, and/or decreased stimulation.
  • Apply restraints; monitor patients in restraints; provide fluids, food, ROM and bathroom; assess and assist with discontinuation criteria.
  • Demonstrate competence in CPI: personal defense releases, blocks, restraint techniques, and escort techniques.
  • Demonstrate competence in verbal de‑escalation skills.
  • Utilize appropriate observation level for patient: Q15 minute checks, Q5 minute checks, line of sight visual observation, and 1:1 observation.
  • When seclusion and/or restraints are used, have patient assessed by a Charge/Staff Nurse and ensure all required documentation is completed.
  • Regularly review work in progress, ensuring that all physician and medication orders are carried out and recorded.
  • Monitor patient for adverse drug reactions and report immediately to Charge Nurse, physician or pharmacist.
  • Ensure medication consents are obtained and signed prior to administering psychotropic medications.
  • Inform Charge Nurse when patient refuses medication.
  • Practice the “5 Rights” of medication administration; know side effects, normal dosage, contraindications of all medications given.
  • Limit medication errors to no more than four per calendar year.
