Overview

Schedule:
Full time - 40 hours/week, Monday - Friday, no weekends/holidays/on-call required, 8:00am – 4:30pm.
This position supports our PHP/IOP - Outpatient clinic.
ResponsibilitiesThe registered nurse (RN, RN II, RN III) performs professional nursing activities to help patients achieve or regain, and then maintain, maximum physical, emotional, or social functions. Role functions are governed by the Nurse Practice Act, the Administrative Code (state of employment), and Rogers Memorial Hospital policies. The RN seeks consultation with other health team members as needed and, in conjunction with Patient Accounts, provides patient information to ensure compliance with federal and state statutes.
- Complete initial assessments and document as required.
- Collect, record, and analyze pertinent data for admission assessment according to Hospital policy, including: patient strengths and limitations, cultural/spiritual/ethnic factors, discharge needs, medical/physical status, medication history and current schedule, and age-specific data.
- Involve patient’s support systems (family, friends) in assessment and documentation; observe and document interactions pertinent to treatment; obtain assessment data when appropriate.
- Act as an advocate for patients: explain rights, obtain signatures, provide information, protect privacy and confidentiality, assist in patient orientation, and ensure compliance with policies.
- Initiate and update treatment plans; participate in planning and modifying the plan of care; evaluate data and participate in care conferences; write clear treatment goals and update as indicated.
- Identify, interpret, and document information from nursing interviews, observation, assessments, and diagnostics; confer with health care professionals as appropriate.
- Review current lab data, assess fall risk, identify safety concerns, and develop age-appropriate interventions.
- Implement patient care, including safe medication administration and patient education; maintain knowledge of medications and reactions; transcribe physician orders accurately; monitor for ADRs and interactions.
- Identify health education needs prior to discharge and implement age-appropriate teaching; document in patient record and treatment plan; organize care activities to meet priorities and time constraints.
- Ensure admissions/transfers comply with EMTALA when applicable; adhere to nursing and hospital standards of care; maintain safety and infection control practices.
- Participate in continuing education and in-service programs; attend required sessions and contribute to Performance Improvement initiatives.
- Assist in development and implementation of unit standards of care and participate in unit committees or projects; lead initiatives as assigned.
- Carry out leadership functions in patient care, staff operations, and department organization; coordinate unit activities and crisis management as charged; supervise and delegate tasks to LPN/psychiatric technicians; provide acuity data for billing; maintain a primary caseload and ensure continuity of care.
- Engage in professional development projects and opportunities to improve skills and unit systems; set goals and pursue ongoing education.
- Promote department goals and the hospital mission; communicate goals and demonstrate measurable achievement; comply with external agency requirements (e.g., Joint Commission, state).
- Demonstrate understanding of Joint Commission and regulatory requirements; participate in trainings and educate other disciplines about department regulations.
- Participate in hospital committees, PI teams, and projects; demonstrate punctuality and effective communication; contribute to solutions in a professional manner.
- Promote performance improvement initiatives; understand and apply PI processes; participate in or lead PI teams to drive improvement across hospital areas.
