Admissions Nurse

Valley View Health and Rehabilitation specializes in providing a "home-like environment" with professional nursing and therapy services for both long-term care and rehabilitation. This means you will be working with residents over an extended period, allowing for the development of deeper relationships and a more comprehensive understanding of their needs. We are seeking a skilled and compassionate Admissions Nurse to join our team.

The Admissions Nurse will play a key role in welcoming new residents, coordinating admissions, and ensuring a smooth transition into our facility. This position requires excellent clinical skills, attention to detail, and a commitment to resident-centered care.

• Conduct initial assessments of new residents to determine care needs and develop individualized care plans.
• Coordinate with families, physicians, and other healthcare providers to gather necessary medical information and ensure seamless admissions.
• Educate residents and families about the facility’s services, policies, and care processes.
• Document resident information accurately in compliance with state and federal regulations.
• Collaborate with the interdisciplinary team to ensure continuity of care.
• Provide emotional support to residents and families during the transition process.
• Participate in quality improvement initiatives to enhance the admissions process.

• Current Registered Nurse (RN) or Licensed Practical Nurse (LPN) license in [State].
• Minimum of [1-2] years of nursing experience, preferably in long-term care or geriatrics.
• Strong assessment and documentation skills.
• Excellent communication and interpersonal skills.
