Position Overview

The Care Gap Program Coordinator supports the Health System Care Gap team in managing, processing, and enhancing gap closure programs for health system customers. A key component of this role is working with large, complex data sets to ensure accurate program setup, patient eligibility verification, and downstream operational readiness. The coordinator partners closely with internal teams—including Revenue Cycle, Customer Care, Customer Success (CSMs), Customer Experience (CX), and the Lab—to intake, validate, and coordinate new care gap program orders.

This includes analyzing customer-submitted files, identifying data quality issues, interpreting business rules, and transforming raw data into actionable inputs that support accurate ordering, communications, and program execution. Additionally, this role will take on sole ownership of specific responsibilities that support the execution & tracking of individual care gap programs and/or our program as a whole.

Essential Duties and Proven Skills
