. Transitions At Home is currently seeking a dedicated and licensed Occupational Therapist to provide in‑home therapy services in the Dodgeville area. This part‑time role focuses on helping clients regain independence and improve daily living skills through personalized, goal‑driven care plans.

Responsibilities

A Home Health Occupational Therapist helps patients regain independence by improving their ability to perform daily activities such as bathing, dressing, and meal preparation. They assess each patient’s home environment, develop personalized therapy plans, and provide hands‑on treatment to enhance safety, mobility, and quality of life. All services are performed in accordance with physician orders and under the direction and supervision of the Director of Clinical Services.

Hours
  • 20-29 hours per week, 8:00 am – 4:30 pm
  • Monday through Friday
  • No holidays required

Qualifications
  • Active Wisconsin Occupational Therapist license
  • Two (2) years’ experience preferred
  • CPR certification
  • Valid Wisconsin driver’s license and car insurance
