We’re looking for a dedicated and licensed Occupational Therapist to provide in‑home therapy services to patients in Madison, WI. This full‑time role focuses on helping clients regain independence and improve daily living skills through personalized, goal‑driven care plans.
What does a Transitions at Home Occupational Therapist do?A Home Health Occupational Therapist helps patients regain independence by improving their ability to perform daily activities such as bathing, dressing, and meal preparation. They assess each patient’s home environment, develop personalized therapy plans, and provide hands‑on treatment to enhance safety, mobility, and quality of life. This is performed in accordance with physician orders and the plan of care under the direction and supervision of the Director of Clinical Services/Clinical Manager.
Hours- 32‑40 hours per week, 8:00 am – 4:30 pm
- Monday through Friday
- No holidays

Job Requirements
- Active Wisconsin Occupational Therapist license
- Two (2) years’ experience preferred
- CPR certification
- Valid Wisconsin driver’s license and car insurance
- Hiring manager will contact you within 1–2 business days
- Complete 1–2 interview(s) and meet with immediate supervisor and mentor
- Upon receiving the job offer, review job description and complete new‑hire tasks and onboarding paperwork (via the Paycor app)
