• Perform psychosocial assessments and develop individualized care plans.
• Provide counseling to residents and families and advocate for residents’ rights and needs.
• Facilitate communication between residents, families, and staff.
• Coordinate discharge planning and ensure compliance with regulations.
• Assess needs, plan, and manage all aspects of the comprehensive activities program.
• Get to know each resident individually to better understand needs and interests.
• Complete screens and orientate residents and families to Oak Park Place.
• Help residents and families adjust to the center.
• Assist families and residents with financial questions and referrals.

• Associate degree preferred.
• Previous experience in a supervisory role.
• Previous experience working with seniors is preferred.
• Strong organizational and planning skills required.
• Ability to work independently is a must.
