This position is for an inpatient clinical pharmacist for our med/surg patient population. Inpatient clinical pharmacist are accountable for medication safety, evidence-based medication use, control of medication cost, improving discharge throughput, and preventing medication related re‑admissions. Vital job functions include admission medication histories; medication reconciliation; discharge medication access and teaching; rounding; nurse and provider education; and medication order review.
Schedule includes a rotation of AM and PM shifts. 1 in 4 weekend rotation and holiday rotation.
