Day Shift Monday‑Friday 8:30 am‑5:00 pm

On‑call
: 1 weekend/quarter, weekend (every 8th), holiday (2/year) rotations |
Sign‑on bonus
: $6,000

How You’ll Make a Difference

As a Nurse Case Manager (RN) you will provide compassionate, patient‑centered care through nursing assessments, case management, education, and interprofessional collaboration, ensuring high satisfaction for patients and families.

Key Responsibilities
  • Patient and Family Assessments
    • Assess physical, pain, psychosocial, emotional, spiritual, environmental, and educational needs.
    • Collect and interpret data from patients, families, and the interdisciplinary team.
    • Document significant changes and report to appropriate team members.
    • Collaborate with providers to determine hospice eligibility.
  • Patient Visits
    • Respond to acute needs, provide interventions, pain management, and education.
    • Evaluate and adjust treatments and education for effectiveness.
  • Plan of Care
    • Develop, implement, and revise individualized plans of care with the IDT.
    • Set realistic outcomes and monitor progress in the electronic health record.
    • Delegate visits and supervise CNAs and LPNs.
  • Miscellaneous
    • Participate in service planning meetings.
    • Precept new staff and students.
Qualifications
  • ADN or BSN required.
  • Current, unrestricted RN license in the state of employment.
  • Preferred one year of nursing experience.
  • Valid driver’s license, auto insurance, and reliable vehicle.
  • At least 18 years old.
  • Basic Microsoft Office and intermediate EHR skills.
  • Knowledge of proper body mechanics, patient handling, and standard precautions.
