Job SummaryOur client is seeking an RN Admissions Nurse to join their team. In this role, you'll take the lead on managing the admissions process for their Skilled Nursing unit and perform assessments for their Assisted Living and Memory Care residents.
Job Responsibilities- Coordinate and manage the full admissions process, including partnerships with healthcare networks and discharge planners
- Collaborate with the Community Liaison and facility staff to ensure a seamless admission experience
- Prepare rooms, paperwork, and support systems for incoming residents across long‑term care, memory care, and rehab units
- Conduct nursing assessments for new residents in Assisted Living
- Partner closely with the Licensed Nursing Home Administrator and interdisciplinary team to facilitate smooth transitions
- Complete all RN admission assessments and documentation in a timely and accurate manner
- Ensure all physician orders, notifications, and care plans are properly handled
- Monitor care delivery and resident outcomes, using established tools and standards
- Address and resolve resident and family concerns in a timely, compassionate manner
- Participate in an emergency on‑call rotation
Complete and document weekly assessment tools
Qualifications- Valid Wisconsin RN license.
- Admissions experience preferred but willing to train the right individual.
- Background in a supervisory role within a skilled nursing facility.
Strong planning, organizational, and leadership skills, with a commitment to quality care.
