Qualifications
  • Graduate of an accredited vocational school of nursing
  • Current Minnesota LPN license (or equivalent)
  • Previous experience in Long Term Care, Home Health, acute care nursing, or Hospice (preferred)
  • Ability to collaborate with others
  • Strong communication and human relation skills
  • CPR certification
  • Written and verbal communication skills
  • Valid drivers license (required)
  • Computer literacy
Responsibilities
  • Perform patient visits under the supervision of an RN Case Manager
  • Provide patient care as delegated by the RN
  • Monitor nursing care provided by other personnel
  • Assist with data collection and plan of care development
  • Utilize iPad for client documentation
  • Adhere to clinical nursing practice standards and the Nurse Practice Act
