How You’ll Make a Difference (Essential Functions)The NP, Hospice/Palliative provides community‑based specialty palliative care to patients and their families while working in collaboration with a patient’s other health care providers and caregivers. This includes the evaluation and management of symptoms and syndromes of advanced disease states, and skillful communication concerning a patient’s diagnoses, disease status, prognosis, care goals, treatment options, and care planning. This position also provides specialty palliative care education and guidance to community health care providers, Agrace employees, and long‑term care facility staff.
The NP, Hospice/Palliative provides care through a consultative model,venue as part of an interdisciplinary team, and participates in ongoing quality assessment and practice improvement, ensuring exceptional patient‑ and family‑centered care.
- Provide clinical care in the community‑based Palliative Care Program: perform telehealth or in‑person initial palliative care consultation including gathering history, performing a physical exam, evaluating medication list, interpreting diagnostics, and providing recommendations for palliative management of a patient’s medical condition in context of their care goals, preferences, and values.
- Provide follow‑up in person or via telehealth to address the palliative care needs outlined in the referral or identified at the initial visit.
- Lead discussions on care goals, preferences, and values with patients, their families, and others involved in their care. Discussions may include sharing difficult news, providing supportive listening and counseling, evaluating quality of life, discussing trade‑offs and time‑limited trials, addressing conflict, and navigating end‑of‑life issues.
- Provide recommendations to patients, families, and others involved in the patient’s care on completingátt атәы ?? on advancing directives in context of the patient’s medical condition and care goals, preferences, and values.
- Facilitate the provision of interdisciplinary care by working together with other disciplines on the palliative care team.
- Communicate and coordinate care with the patient, family, caregivers, and the referring physician and/or Primary Care Physician (PCP).
- Participate in on‑call rotation to support the hospice team.
- Discuss medication/therapy needs for hospice patients with RN.
- Decide on medication changes/refills/treatments; submit orders to the pharmacy.
- Collaborate with physician on call regarding complex patient needs.
- Complete face‑to‑face evaluations and participate in incapacity assessments, as appropriate, when needed to meet hospice patient needs.
- Participate in internal work groups, meetings and committees relative to position responsibilities.
- Ensure compliance with Agrace policies and protocols, state, federal, and other regulations.
- Precept new staff and students.
- Valid driver’s license, auto insurance, and access to a reliable vehicle.
- Current, unrestricted Advanced Practice Nurse Prescriber licensure, and NP certification in state of employment; DEA licensure.
- Will require Wisconsin, Minnesota, Iowa licensure within 3 months of hire.
- Telehealth experience preferred.
- CPR certification (required within 90 days of hire).
- 2+ years’ experience as a Nurse Practitioner, preferred; hospice/palliative care experience, preferred.
- Advance Certified Hospice and Palliative Nurse (ACHPN) certification, preferred.
- Experience with and knowledge of health care policies and reimbursement practices, state and federal health care regulations and home health standards and procedures.
- Experience with personal computers, database applications, and Microsoft Office software. Experience with electronic health records.
