Triage RN: Care Coordination & Phone Triage

Vail Health is seeking a Triage Registered Nurse to perform care coordination and triage functions in a dynamic health system. You will assess patient needs, triage calls and portal messages, and support providers within a care coordination team.

Responsibilities include documenting assessments, following policies, and contributing to quality improvement initiatives. Requires an RN license and 3+ years of experience;
Colorado license or compact state license is needed.
