JOB SUMMARY:
We are seeking compassionate professionals to join our team as Case Managers for the Children's Long-Term Support (CLTS) Waiver Program in Dane County. As a Case Manager, you will provide service coordination services to youth and families needing counseling and referral services for one or more of the following: intellectual and/or developmental, physical, or mental health disabilities.

KEY RESPONSIBILITIES OF THE CASE MANAGER:
- Receive new client referrals and make initial contact.
- Conduct assessments of client needs through interviews, collateral contacts, and other investigations.
- Maintain an individualized service plan with clients and perform casework services.
- Maintain a caseload involving clients with personal, family, or socio-economic problems and attend to their related protection and safety needs.
- Counsel and provide intervention and case management services by coordinating various social, community, and other services received by the clients.
- Complete in-person home visits to conduct assessments and maintain client contact.
SALARY: $ per hour
LOCATION: Madison, WI. Local travel is required for client home visits throughout Dane County, with mileage reimbursement.
SCHEDULE: Monday-Friday 8:00am-5:00pm, with occasional evenings to accommodate families' schedules.

REQUIREMENTS:
- Must have reliable transportation on a daily basis, a valid driver's license, and automobile insurance.
- Bachelor's degree in sociology, social work, psychology, or closely related is required.
- One year of professional social work experience is required.
