Porchlight, Inc. is committed to reducing homelessness in Dane County by providing shelter, affordable housing, and supportive services that address barriers and empower individuals and families. As Dane County's largest non‑profit provider of affordable housing, Porchlight operates an emergency shelter for men, an eviction prevention program, an outreach team, a job‑training program, and numerous housing and supportive services.

Role Description

This is a full‑time, on‑site role based in Madison, WI. As a Veterans Transitional Housing Program (VTHP) Case Manager, you will provide case‑management services to program participants, including conducting assessments, developing individualized service plans, and offering guidance toward achieving housing stability and self‑sufficiency. The role involves supporting clients in accessing community resources, facilitating skill‑development sessions, and maintaining thorough documentation. Collaboration with other team members and external partners is essential to address client needs effectively.

Qualifications
  • Experience in case management, client assessments, and developing individualized service plans.
  • Strong advocacy skills and the ability to facilitate connections between clients and community resources.
  • Proficiency in communication, organization, and maintaining client documentation.
  • Experience working with diverse populations, including individuals experiencing homelessness or veterans. Understanding of trauma‑informed care is a plus.
  • Familiarity with housing resources and social service programs in the region, particularly in Madison, WI.
  • A Bachelor’s degree in Social Work or a related field is preferred. Equivalent professional experience will be considered.
  • Ability to work collaboratively in a team setting and independently as required.
  • Proficient in basic computer skills such as MS Office and data entry software.
  • Valid driver’s license and reliable transportation are highly desirable.
