Overview

Clinical Staff Pharmacists are responsible and accountable for the provision of safe, effective, and prompt medication therapy. Through various assignments within the department, they provide support of centralized and decentralized medication use systems as well as deliver optimal medication therapy to patients with a broad range of disease states.

All pharmacists are responsible for drug preparation and distribution for medications; evaluating, inputting and verification of medication orders; assisting providers and other health care professionals by serving as a drug information resource; monitoring and managing medication regimens per protocol or per consult; and managing pharmacy inventory.

Principal Duties
  • Serves as a drug information expert and resource regarding medication management
  • Has a working knowledge of pharmacy operations including: pharmacy operating system, use of patient record, purchasing, inventory and formulary management, automation, packaging, USP 797, USP 795 and pharmaceutical waste
  • Initiates, documents, and participates in department activities that promote positive patient outcomes. These activities may include, but are not limited to:
    • Identifying medication process issues and implementing solutions
    • Clinical and cost savings initiatives, therapeutic drug monitoring and appropriate formulary utilization
    • Provides comprehensive pharmacy clinical services including but not limited to: anticoagulation management/monitoring, pharmacokinetic dosing, renal dosing, nutritional support services
    • Participates in antimicrobial stewardship and opioid stewardship
    • Medication usage evaluations
    • Adverse event reporting
    • Working with inter-professional teams to optimize order sets, automated dispensing cabinet inventory, and medication related policies
  • Assists in the orientation and training of pharmacists, technicians, students and residents; aids in performing competencies on staff (new hire and annual) as requested by manager/director
  • Serves as an active member of the inter-professional team on patient care units. Participates in unit-based huddles and activities if available. Knowledgeable in unit’s metrics, goals, and works with staff on unit to help achieve goals
  • Acts as preceptor for both students and residents as applicable
Qualifications
  • Graduate of an accredited College of Pharmacy with a BS in Pharmacy with a Pharm

    D preferred
  • Kentucky Board of Pharmacy license in good standing as applicable to practice site
  • Current Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certification preferred
