Join Optum’s Elite Home Health, part of LHC Group, to provide quality patient care and technical skilled care in patients’ homes.

• Provide high quality clinical services within scope of practice, as defined by state laws, in accordance with the plan of care, and in coordination with other health care team members, following all applicable infection control procedures.
• Document patient visits thoroughly and completely per regulatory and payer requirements in the electronic medical record utilizing the agency's electronic visit verification program (as applicable).
• Actively participate in the agency's care coordination process, including timely reporting of patients' needs and changes in condition, attending patient case conferences, communicating effectively to the patient, family, physician, other clinicians, agency management staff, case coordinator, and community case managers (as applicable).

• Current and unrestricted LVN licensure in state of practice.
• Current CPR certification.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.
• Ability to work flexible hours as required to meet identified client needs.
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.

• Home care experience.
• Ability to manage multiple tasks simultaneously.
• Ability to work independently.
• Good communication, writing, and organizational skills.

Pay is based on local labor markets, education, work experience, certifications, etc. Hourly pay ranges from $20.00 to $35.72 per hour for full‑time employment.
