What you will do:

• – Conduct outcome-focused assessments for adults with physical disabilities and age-related conditions.
• – Perform complex moving and handling assessments within the Front Door Team.
• – Evaluate physical dysfunction and functional ability to recommend appropriate complex equipment solutions.
• – Provide expert manual handling advice and intervention planning.
• – Support safe and effective care planning, managing cases independently.
• – Collaborate with multidisciplinary teams and partner agencies to ensure holistic care.
• – Maintain meticulous records and assessment documentation to support care delivery.

Requirements:

• – Degree or Diploma in Occupational Therapy, with HCPC registration.
• – Proven experience as a Senior Occupational Therapist in an Adult Social Care setting.
• – Extensive experience with complex moving and handling and equipment assessment.
• – Autonomous work capability with strong assessment and clinical reasoning skills.
• – Excellent communication and report-writing skills.

Desirable Requirements:

• – Previous experience within a Front Door Team or community Occupational Therapy setting.
• – Familiarity with Local Authority services and managing complex care assessments.
