Providing autonomous clinical advice, leadership and education to our nursing and residential homes.
Collaborating with GPs, community services, hospitals and internal teams to ensure residents receive the right care, at the right time.
Leading frailty-focused triage and assessment via phone, video and in-person, helping avoid unnecessary hospital admissions.
Mentoring nurses undertaking advanced training and contributing to clinical improvement projects and research.
Developing and maintaining clinical policies, procedures and competency frameworks.
Current registration with the NMC and a degree in nursing.
Registered non-medical prescriber.
Full, current UK driving licence
Proven recent experience in Advanced Clinical Practice within health or social care.
Experience in clinical leadership, non-medical prescribing, diagnostic reasoning and assessment.
Proven ability to manage deadlines, prioritise demands and lead improvement projects.
