You'll deliver personalised care in a community setting, working with nurses, GPs, therapists and social care colleagues to prevent hospital admissions and support early discharge.
A full UK driving licence is essential. You'll provide clinical and personal care, record care details, take basic observations, and support patients to remain safely at home.
