About the Position:

During visits that can last up to one hour, Matrix providers review and observe a member’s current health, medical history, medication adherence, social environment, and other risks. This provides unmatched insight into a member’s overall health and well-being that can be difficult to capture during routine office visits. The Matrix Comprehensive Health Assessment helps improve quality of care and allows us to potentially close multiple care gaps with a single visit.

Responsibilities:

What to Expect:
  • Conduct adult/geriatric assessments including medical history, diagnosis, treatment, health education, referrals, follow-up, and documentation according to Matrix guidelines and protocols.
  • Collaborate with physicians, case managers, social workers, family members, caregivers, and other medical personnel as appropriate.
  • Work with Primary Care Physicians on patient education and follow-up.
  • Provide services in various venues, including home visits and skilled facility visits.
Position Details:

Type: PRN - Flexible Nurse Practitioner or Physician Assistant

Pay: Earn from $1,900 up to $9,500 per month based on 20-100 visits, at $95 per assessment. Compensation varies with availability and assessment completion.

Location: Home, SNF, and other community environments in Millinocket, Maine

Hours: Days, evenings, nights, weekends; flexible scheduling

Benefits: PRN providers are non-benefited except for 401(k) and limited voluntary benefits.

Mileage reimbursement plan included.

Qualifications:
  • Master’s Degree or equivalent experience with NP or PA licensure.
  • Current RN and NP or PA licensure in the state of practice, including prescription authority or eligibility to obtain it.
  • Board certification by NCCPA, AANP, ANCC, or AACCN in a Matrix-approved specialty.
  • Current BLS, ACLS, or CPR certification.
  • At least 1 year of experience preferred; new grads encouraged to apply. Ability to travel in and out of state.
  • Strong computer skills and familiarity with EMR software.
  • Excellent communication skills and a positive, team-oriented personality.
Travel Requirements:
  • Travel to meet patients where they live.
  • Valid driver’s license, ability to drive, and proof of adequate auto insurance.
