Position OverviewAspire Senior Living Malden is seeking a detail-oriented and compassionate MDS / Social Services Coordinator to oversee MDS assessments while also supporting the psychosocial well‑being of our residents. This hybrid role is responsible for ensuring accurate and timely MDS completion, regulatory compliance, and resident-centered care planning, while serving as an advocate and liaison for residents and families.
The ideal candidate combines strong clinical and regulatory expertise with excellent communication and interpersonal skills.
Key ResponsibilitiesMDS & Regulatory
Complete, coordinate, and oversee all MDS assessments in accordance with CMS guidelines.
Ensure timely and accurate MDS submissions to CMS.
Maintain compliance with all federal, state, and local regulations.
Collaborate with interdisciplinary team members to develop, update, and monitor resident care plans.
Audit resident records for accuracy, completeness, and compliance.
Monitor care plans to ensure effectiveness and regulatory alignment.
Assess and address residents’ psychosocial needs upon admission and throughout their stay.
Develop and implement individualized psychosocial care plans.
Serve as a liaison between residents, families, and facility staff to facilitate communication and resolve concerns.
Provide emotional support, counseling, and crisis intervention to residents and families.
Coordinate discharge planning and ensure smooth transitions to the community or other care settings.
Advocate for residents’ rights and promote resident-centered care.
Connect residents and families with appropriate community resources.
Maintain thorough and accurate documentation of assessments, care plans, and interventions.
Organize or support resident and family education programs as needed.
RN or LPN licensure in Missouri required.
SSD Certificate or willingness to obtain.
Previous experience in long‑term care, MDS coordination, or social services strongly preferred.
Strong knowledge of CMS guidelines and federal/state long‑term care regulations.
Proficiency with MDS software and electronic medical records.
Excellent organizational, communication, and interpersonal skills.
Ability to work collaboratively within an interdisciplinary team.
