Assisting Hands Home Care® is seeking a Registered Nurse (RN) in Tinton Falls, New Jersey. This is an opportunity for someone who has a passion for helping seniors and a proven track record of providing exceptional care.

Job Description

The RN Care Coordinator will be responsible for overseeing delivery of all companion and personal care services delivered by the agency. Manage the day to day scheduling of caregivers, cultivate relationships with referral sources and customers.

Responsibilities and Duties
  • Performing client assessments
  • Establish client care plans
  • Coordinating client care with client’s health care team
  • Coordination and supervision of the caregiver staff
  • Maintain appropriate client and staff documentation/records (best practices) as per agency policy
  • Provide agency’s orientation, training and in-service education
  • Make home supervisory visits to assess client status and caregiver performance
  • Prepare performance evaluations for caregiver staff as required
  • Sharing in the on-call rotation after hours as needed for coverage and clinical oversight.
  • Identify referral sources (ex. healthcare facilities and professionals) within the community and surrounding area.
Experience and Requirements
  • Strong clinical and assessment skills
  • Strong interpersonal and customer service skills
  • Excellent communication, organizational and mentoring skills
  • Must be organized, detail oriented, self‑motivated, and able to work independently
  • Ability to handle multiple priorities simultaneously
  • Prior community health experience, long‑term care, or case management experience required.
  • Familiarity with the Monmouth County area a plus but not required
  • Comfortable conducting CEU programs
