Job Summary

The System STEMI Coordinator’s primary focus is on the maintenance and continuous quality improvement of UVA Community Health’s (UVA CH) STEMI practices. The coordinator collaborates with the health team in all aspects of patient care, provides formal and informal program education for providers and team members, and collaborates with nursing staff across the system to facilitate patient care and health management regarding appropriate discharge options.

The coordinator collects and manages clinical, financial, quality, and disease‑specific related care data.

Responsibilities
  • Program Management:
    Serve as a liaison to staff, physicians, and Emergency Medical Services regionally; lead and evaluate the STEMI process; act as a resource to nursing, health management specialists, cardiac rehabilitation teams, and physicians; act as a change agent to achieve program goals and objectives; organize and facilitate STEMI meetings across UVA CH; evaluate interdisciplinary care processes; assist with development and revision of protocols, standards, and educational materials;

    coordinate UVA CH STEMI education programs and community STEMI events; plan and implement STEMI education tools, presentations, and screenings to increase STEMI/CARDIAC awareness in the community; plan and implement activities for American Heart Month.
  • Data Management:
    Utilize compliance databases specific to the desired patient population; compile data to establish profiles for state and national comparison; support evidence‑based practice; assess educational needs of the inpatient and outpatient STEMI population and develop educational materials/tools to facilitate learning; manage data from STEMI logs and ensure compliance with data entry and analysis for participating databases.
  • Patient Care:
    Work with team members to develop or improve processes to assure a remarkable patient experience; create and maintain a healthy work environment that results in improved patient outcomes, retention, and organizational excellence; collaborate with physicians to develop or improve processes surrounding care of patients.
  • Quality:
    Work in partnership with leaders to create, support, and maintain a culture of professional accountability; understand the value of innovation and quality improvement; improve processes and practices by identifying inefficiencies and redundancies; collect, analyze, and display qualitative and quantitative data to identify and understand opportunities for improvement; demonstrate efficiency and quality in one’s own work; assure that team members provide care consistent with The Joint Commission and other regulatory requirements.
  • Communication:
    Consistently communicate in an efficient and effective manner appropriate to the target audience through professionally developed interpersonal skills and utilize clear oral and/or written communication tools as indicated; collaborate with physicians to improve processes surrounding care of patients; connect leaders and front‑line team members to resources to enhance improvement efforts; facilitate program‑specific and performance improvement‑focused interdisciplinary meetings; create an environment that supports and respects differences and belonging;

    coach and mentor the team to achieve goals.
