Licensed Practical Nurse, Ambulatory - Cardiology

The Licensed Practical Nurse (LPN) provides care within UVA Health Northern VA & Culpeper, collaborating with an interprofessional team to promote optimal health. The LPN works under the direction and supervision of an RN/LIP and/or Clinic Leader, serving all age populations with developmentally appropriate care. Must demonstrate evidence-based practice, compassion, and accountability in patient care.

Job Summary

The Licensed Practical Nurse serves as a member of the healthcare team by establishing authentic, personalized relationships with patients and their chosen support systems, and by collaborating with other team members to promote optimal health. The LPN uses best scientific evidence and compassion to assist in the patient’s health journey.

Key responsibilities include providing and documenting patient care under supervision as assigned.

Responsibilities
  • Patient Care:
    Provides assessment, evaluation, and management of patients.
  • Participates in the collection of patient data as needed.
  • Determines nursing care needs by recognizing relationships between data and the patient’s health status and treatment regimen.
  • Identifies relevant resources based on necessity and availability.
  • Communication:
    Communicates information to others responsible for or involved in the care of the patient.
  • Engages with the patient and chosen support system to identify the individualized care plan.
  • Utilizes SBARQ when communicating within the interdisciplinary team and during handoffs.
  • Documents appropriate information in a timely manner.
  • Teamwork:
    Participates as part of the interdisciplinary care team in planning individualized care.
  • Collaborates with the registered nurse to modify the care plan based on patient needs, capabilities, and condition.
  • Medical Knowledge:
    Demonstrates core knowledge of biomedical and clinical sciences and applies it to patient care.
  • Identifies nursing interventions and goals, and reviews goals with the RN.
  • Identifies opportunities for process improvement, evidence-based practice, and research.
  • Engages in self-reflection and develops personal goals to enhance knowledge and pursue certification or further education.

The incumbent may be asked to perform additional duties as assigned.

Qualifications
  • Education: High School Diploma required. Graduate of accredited Practical Nursing Program required. Completion of a nursing program approved by the State Board of Nursing.
  • Experience: 1 year of relevant experience required. Refer to Life Support Training Policy for details.
  • Licensure: Applicable state LPN license required.
  • Additional Skills/Requirements

    Required:

    Ability to complete general and department-specific skills validation and competency testing. Ability to interpret information to identify each patient’s care requirements relative to age-specific needs.

  • Constant standing, frequent walking, bending/stooping. Proficient communication, auditory, and visual skills. Attention to detail and legible handwriting. Body capable of lifting/pushing/pulling up to 100 lbs. Use Safe Handling Equipment for weights over 35 lbs.
