Job Title: Case Manager (Second Shift) at Community Health Resources
Employment Type: Full-time, 40 hours/week
Schedule: Monday–Friday 3:00 pm–11:30 pm (Second Shift)
Location: Strickland, Manchester, CT
Program: Strickland – an intensive residential program providing 24‑hour support for individuals discharged from CVH.
Responsibilities- Utilize appropriate therapeutic models to help clients understand and manage symptoms across environments.
- Conduct program‑specific assessments and required screenings.
- Provide case management, collaborating with clients’ natural supports and treatment teams, connecting to prosocial activities, and delivering crisis intervention and de‑escalation as needed.
- Demonstrate competency in age‑related or specialty issues and developmental needs for the target population.
- Teach and support learning of emotion‑regulation, age‑appropriate activities, and home‑like living skills; plan and participate in related activities.
- Document all services, contacts, and appointments in compliance with agency standards.
- Transport clients as program requires.
- Participate in staff meetings, supervision, training, and other assigned meetings.
- Perform additional duties as required.
Education: Bachelor’s degree in human services preferred; education plus equivalent work experience required.
Experience: At least two (2) years in a mental health setting preferred.
Licensure / Certification: Valid driver’s license.
CompensationMinimum $18.86 per hour, commensurate with experience.
Rates determined at offer time and may include program/location differentials or sign‑on incentives.
