** Position Summary*
* The Population Health Nurse is responsible for reviewing and implementing clinical workflows within ECMP physician practices to achieve PCMH level 3 recognition. This role involves identifying vulnerable populations using data from visits, claims, labs, medications, and hospital discharges. The nurse collaborates with clinical staff, physicians, quality committees, and IT to improve outcomes for patients with chronic diseases by coordinating healthcare services tailored to individual needs.

Minimum Requirements
  • Graduate of an accredited nursing school; associate degree required, bachelor’s preferred.
  • Current license as a Licensed Practical Nurse in Connecticut.
  • At least five (5) years of nursing experience in a community practice working with vulnerable populations.
Responsibilities
  • Collaborate with healthcare teams to identify and support vulnerable patients.
  • Support physicians in developing individualized care plans.
  • Review and implement PCMH standards and processes.
  • Participate in research and planning for future standards.
  • Ensure clinical standards adherence and support workflow changes.
  • Lead clinical quality initiatives and participate in team meetings and committees.
  • Maintain patient confidentiality and HIPAA compliance.
Qualifications and Skills
  • Strong analytical, organizational, and communication skills.
  • Proficiency in Microsoft Office.
  • Ability to multitask and work independently.
