Our case manager provides mental health services to children and adolescents as a part of a office/community‑based treatment team. The position provides an opportunity for rewarding work with families in need of mental health support, consultation with dedicated colleagues, collaboration with community partners, and growth opportunities.

• Case management including an assessment of the needs of the child/adolescent and family
• Active collaboration with community providers and services
• Utilize therapeutic skills
• Collaboration with multidisciplinary teams
• Developing treatment plans, completing clinical notes and other documentation utilizing an electronic medical record

Bachelor’s degree in psychology, social work, or education/counseling from an accredited college or university.

• Commitment to teamwork
• Self‑Starter, Independent
• Time management and organizational skills are a must.
• Excellent communication ability with co‑workers, staff, clients, and community partners.

Must possess a valid driver’s license and automobile with liability insurance in the amount required by agency policy.
