** About the Job:
** The Home Health and Hospice Admission Liaison will build, establish, and maintain strong referral relationships with Skilled Nursing Facilities (SNF), nursing facilities, assisted living facilities including staff and physicians ensuring all patients who would require and benefit from VNA services have access to those services.  The incumbent will coordinate transitions to the agency; will admit patients to hospice residing in facilities;

will provide in-services to facilities related to hospice care; and will promote and educate about all VNA programs and services.

* What You’ll Do:

*** Assist with access and transitions of care to Homecare and hospice by building long-term relationships and ensuring excellent customer service. Ensure the agency is a preferred provider. Facilitate cooperation between and among facility and Hospice staff.
* Determine appropriateness of Homecare referrals; gather pertinent clinical information and coordinate and conduct preadmission activities for Homecare.
* Determine appropriateness of hospice referrals; coordinate and conduct preadmission activities for hospice.
* Conduct hospice admission on-site cuss patient with medical director and complete clinical documentation in accordance with agency policy.
* Communicate with IDG and case manager regarding new admission to ensure appropriate clinical care and handoff of patient care to case manager.
* Educate facility staff regarding Hospice principles and Hospice philosophy. This includes familiarizing facility staff with all policies and procedures related to the agreement and to the provision of Hospice Services to Facility Residents who elect hospice care; familiarizing facility staff with the forms, documents and record keeping requirements in use by Hospice and by the Facility in caring for residents;

familiarizing staff with methods of comfort, pain control and symptom management used by Hospice; familiarizing facility staff with Hospice in-patient protocols and patient and family rights; assisting facility staff in identifying their own feelings, reactions, strengths and limitations in dealing with dying persons, and with death and dying; and developing strategies for meeting staff members' needs in caring for terminally ill individuals, and to clarify support, in-service education and supervision available to assist with meeting those needs.
** Who You Are:

* Education:

** Graduate of an accredited school of nursing; BSN or equivalent preferred.  
** Licensure/certification:
** Current licensure in the state of New Hampshire as a Registered Nurse required. Valid driver’s license required.

* Experience:

** A minimum of one to two years Hospice or Homecare Nurse experience preferred. Experience in preparing and providing education programs preferred. Sales and Marketing experience preferred.
