About the RoleWe are seeking an enthusiastic, compassionate, and dedicated SEMH Support Practitioner to join the Base 12 team.
Base 12 is a specialist programme designed to support young people aged 16-25 with an Education, Health and Care Plan (EHCP) for Social, Emotional and Mental Health (SEMH) needs. The programme helps learners who are not yet ready to progress directly onto a vocational college course by developing their confidence, resilience, independence, employability, and social skills.
Working closely with teaching staff and the Pastoral & Wellbeing Coordinator, you will provide pastoral, behavioural, and learner engagement support both inside and outside the classroom. You will play a key role in helping learners overcome barriers to education, achieve their personal goals, and successfully progress into further education, training, or employment.
Key Responsibilities- Provide pastoral, emotional, and behavioural support to learners with SEMH and additional learning needs.
- Build positive and professional relationships with learners, acting as a trusted mentor and role model.
- Support learners to develop confidence, resilience, independence, and social skills.
- Encourage learner engagement, attendance, participation, and positive behaviour.
- Deliver pre-planned group tutorials and personal development sessions.
- Support learners during vocational, enrichment, and employability activities.
- Assist learners in working towards their academic, personal, and progression targets.
- Liaise effectively with teaching staff, support services, parents/carers, and external agencies where required.
- Experience working with young people aged 16-25 within education, youth work, mentoring, support work, or SEND environments.
- Knowledge and understanding of SEMH, SEND, EHCPs, and barriers to learning.
- Ability to build rapport and maintain positive relationships with young people from diverse backgrounds.
- Experience managing challenging behaviour using positive and restorative approaches.
- Strong communication and interpersonal skills.
