Millgate Healthcare Centrein Denton is seeking a motivated and enthusiastic individual to join our team for a 4-day-per-week position. The role can be split between two people working two days a week each. We are a busy but friendly GP practice committed to providing high-quality patient care to the local community. About the Role:

This role will primarily focus on managing on-the-day acute presentations, providing a valuable service to our patients and ensuring they receive timely and effective care. You will be a key member of our clinical team, utilising your advanced skills and knowledge to assess, diagnose, and treat patients presenting with a range of acute conditions.

We are looking for someone who:

  • Is a qualified and experienced Nurse Prescriber or Advanced Nurse Practitioner
  • Has experience in managing acute presentations within a primary care setting.
  • Possesses excellent clinical skills and decision-making abilities.
  • Is a strong team player with excellent communication and interpersonal skills.

Job responsibilities
  • Develop, implement and embed health promotion and wellbeing programmes
  • Manage patients presenting with a range of acute and chronic medical conditions, providing subject matter expert advice
  • Implement and evaluate individualised treatment plans for chronic disease patients
  • Identify, manage and support patients at risk of developing long‑term conditions, preventing adverse effects to the patient’s health
  • Provide advanced, specialist nursing care to patients as required in accordance with clinical evidence, NICE and the NSF
  • Provide wound care (ulcer/Doppler, etc.)
  • Provide specialist clinics such as ear syringing, contraception, etc.
  • Undertake the collection of pathological specimens
  • Request pathology services as necessary
  • Process and interpret pathology and other test results as required
  • Provide chronic disease clinics, delivering patient care as necessary, referring patients to secondary/specialist care as required
  • Maintain accurate clinical records in conjunction with extant legislation
  • Ensure SNOMED CT codes are used effectively
  • Maintain chronic disease registers
  • Develop, implement and embed well woman clinics
  • Develop, implement and embed well man clinics
  • Develop, implement and embed travel clinics
  • Develop, implement and embed an effective call/recall system
  • Be responsible for the implementation of an effective immunisation programme
  • Chaperone patients where necessary
  • Prioritise health issues and intervene appropriately
  • Support the team in dealing with clinical emergencies
  • Recognise, assess and refer patients presenting with mental health needs
  • Implement vaccination programmes for adults and children
  • Be an extended and supplementary prescriber, adhering to extant guidance
  • Support patients in the use of their prescribed medicines or over‑the‑counter medicines (within own scope of practice), reviewing annually as required
  • Contribute to practice targets (QOF, etc.), complying with local and regional guidance
  • Liaise with external services/agencies to ensure the patient is supported appropriately (vulnerable patients, etc.)
