Manchester Locality - Pharmacy Technician - Fixed-term to cover maternity leave.
Working across NHS GM or a designated locality on joint work streams and initiatives and promoting awareness of Medicines Optimisation issues. This role will require direct travel to GP practices and office locations.
To promote, facilitate safe and effective high quality and cost-effective prescribing and medicines management in base locality and across Greater Manchester.
Providing technical assistance, working within GP practices to implement and maintain high-quality, cost-effective changes in prescribing trends and reducing medicines-related risks to patients.
Act as one of the link persons for specified GP practices to implement medicines optimisation initiatives in line with the primary care, and medicines optimisation team priorities.
To undertake prescribing audits and work within Standard Operating Procedures (SOPs) and support the development of repeat prescribing systems within GP practices as directed by line manager.
Support with the management of Medicines Information enquiries within GP Practice, social care settings or as part of a medicine's information service.
Complete the reconciliation of medicines detailed in patient medical documents.
Using prescribing data, information, knowledge and technology to contribute to the management of the prescribing budget and improve, inform and support a portfolio of Medicines Optimisation projects, services, and initiatives.
Main duties of the jobTo identify implement and deliver on Medicines Optimisation and primary care quality and CIP objectives.
To work on GP practice-based, prescribing, primary care initiatives and standards, to enable practices to achieve targets whilst optimising medicines optimisation.
To build and maintain strong relationships and be a key member of teams outside of own organisation
Regularly produce, analyse, interpret and present financial & clinical prescribing data at practice, PCN, locality and Greater Manchester level, enabling the targeting of support and monitoring of targets.
Develop, implement and complete clinical and non-clinic audits to support Medicines Optimisation objectives and improve prescribing quality.
To contribute to the development and implement Standard Operation Procedures, guidelines, policies & protocols for own locality.
To provide information, advice and query answering in relation to general medicines management issues
To carry out medication optimisation reviews in a domiciliary, care home or clinic setting meet with vulnerable patients and carers referred by other healthcare or social care professionals or identified during audit.

responsibilities
Communicate complex and sensitive financial information to prescribers and practice managers about their performance against budgets, standards and targets.
Support & contribute to the development and of internal and external communications, such as prescribing alerts by regular contact with the teams, stakeholders and Communications team.
Demonstrate advanced computer skills using a range of Microsoft (MS) applications, including but not exclusive to Word, Excel, Outlook, MS Teams and PowerPoint.
Regularly use advanced computer skills to utilise GP clinical systems to gather, analyse and interpret patient related prescribing and clinical data. To supplement this with information from patient's Lloyd George paper records if needed, in order to undertake clinical audits arising from the primary care, Medicines Optimisation work streams and priority areas in line with current local and national guidelines. Requires the ability to identify errors and/or near misses as well as identifying opportunities for optimisation of medicines.
