Band 7 – Senior Mental Health Practitioner

An exciting opportunity has arisen for a Band 7 Senior Mental Health Practitioner to work for the South Manchester Home-Based Treatment Team based at Laureate House Wythenshawe Hospital.

The post holder will work collaboratively with the Team Manager, and Senior Mental Health Practitioners, and colleagues across Urgent Care Services, Mental Health Liaison, Home Treatment Teams, inpatient wards and the Patient Flow Service, to deliver the Trust’s aims of providing effective care, support, and recovery to individuals with serious mental health problems who are experiencing a crisis in their mental health.

The post holder will support individuals to progress safely and effectively through urgent care pathways, promoting recovery, reducing unnecessary delays, and ensuring person centred care throughout their journey.

The post holder will contribute to the maintenance and monitoring of high standards of urgent care including service quality, governance, and performance within the team, whilst ensuring the Trust’s vision, values, and principles are consistently upheld.

The post holder will be required to work 37.5 hours a week including shifts, weekends, and bank holidays This service operates over 24 hour, 7 days a week. There is an expectation that the post holder will participate in out-of-hours Operational on call this is subject to approval.

To develop effective partnerships with primary care colleagues and non-statutory service providers.

To deliver the Trust’s aims of providing effective care, support and recovery to individuals with serious mental health problems who are experiencing a crisis in their mental health.

To provide leadership and management for all staff in the HBTT, enabling the delivery of a flexible service that responds to the needs of service users and their carers/ families.

To contribute to the maintenance and monitoring of high standards of urgent care including service quality, governance and performance within the team and ensuring the Trust’s vision, values and principles are upheld.

To contribute to the management of team processes for accepting and allocating referrals and ensuring effective and planned interventions are delivered.

To ensure individuals are moved through the urgent care pathways effectively and supported in their recovery.

To support the HBT Team Manager in leading and managing the HBTT (including deputising for the Manager in their absence)

To undertake assessment, care planning, therapeutic intervention and review of care for individuals who are experiencing a crisis as well as providing an advisory resource to colleagues.
