Community Care Live is seeking a Locum Social Worker to join a brand new Review Project under the Learning Disability Team. This role requires a Social Work qualification with a minimum of 3 years post-qualified experience and a valid UK driving licence.
You will undertake detailed Care Act reviews and reassessments for adults with learning disabilities, contact families, advocates, and health professionals, and produce audit-ready assessments and care plans for multifaceted cases.
