Job Summary Advanced Clinical Practitioner- Manchester City Centre
Full-time Advanced Clinical Practitioner £55,157 to £68,544, yearly salary increments, NHS Pension provider, rate enhancements for out of hours working.
gtd healthcare is one of the largest NHS commissioned Primary Care, Out of Hours & Urgent Care providers in the North West of England. We have fantastic opportunity for Advanced Clinical Practitioner to join our City Health Centre Walk-in Service in Manchester City Centre.
Our City Health Centre Walk-in Service is situated above Boots on Market Street, which offers a great selection of cafes, restaurants and shops with excellent transport links. We operate 08:00 to 20:00, 7 days per week. Patients can access the service by walking in, via NHS 111 or by referral from other healthcare services.
The successful candidate will play a key role in delivering safe, effective and responsive care, ensuring patients receive timely assessment, treatment and onward referral where appropriate.
The successful candidate will have experience working in primary and urgent care as an ACP, you will be expected to provide care via face to face, and telephone.
The post holder will hold an MSc in Advanced Clinical Practice or Credentialing qualification (or equivalent) and a Non-Medical Prescribing qualification. You will be able to work within the four pillars of practice.
Main duties of the job- Undertake the holistic assessment and clinical examination of patients, of all ages, with both minor and complex needs will be established through history taking, physical examination and the appropriate use of investigations to confirm diagnosis.
- Plan, implement and evaluate an effective management plan for the patient, ensuring safe discharge/referral and/or transfer to an appropriate health care service.
- Provide information and advice on prescribed or over-the-counter medication regimens, including side-effects and interactions.
- The prioritisation of health problems and the appropriate intervening will be paramount in appropriately assisting the patient in complex, urgent or emergency situations, including initiation of effective emergency care.
- Refer and discharge safely ensuring that safety-netting advice is offered.
- Triage telephone referrals, calls from external clinicians, patients and others into a range of services appropriately as required.
