Home from Hospital is our hospital discharge support service. You'll be part of a team doing valuable work to support vulnerable patients who have been discharged from Manchester's hospitals (in the Manchester City Council area).PURPOSE OF JOB

Manage the day-to-day running of an efficient, high-quality, person-centred Home from Hospital Service, with referrals from North Manchester General Hospital, and Community Health Care services
Ensure the practical and social support needs of vulnerable patients being discharged from Hospital are met to a high standard and within agreed timescales
Manage referrals to staff undertaking home visits and ongoing telephone support for patients
Work with the Age UK Manchester Home Care team and other care professionals and volunteers to ensure patients are supported in maintaining and improving their independence and quality of life following discharge from hospital
Act as a representative from Age UK Manchester when liaising with Hospital staff and other agencies
Ensure all safeguarding concerns are identified, recorded and reported in line with organisational policies and procedures, working closely with statutory agencies where appropriate.
Ensure that the Equal Opportunities, Health and Safety and Confidentiality policies of Age UK Manchester, as they apply to the service, are carried out properly
To work with other members of the team to improve the effectiveness of Age UK Manchester, providing relief cover for other staff if and when required

TASKS

Take referrals for patients being discharged from North Manchester General Hospital and Community Health Care Services. For a time-limited package of practical and social support
Coordinate and oversee the daily allocation of referrals and workload across the Home from Hospital team to ensure timely and effective service delivery.
Liaise with health professionals and care managers to ensure that comprehensive practical support designed to promote the physical and mental health of the patient is provided and to encourage the patient to maintain an independent lifestyle
Arrange for a member of the Home from Hospital team to visit discharged patients in their home and for their immediate practical and social support needs to be seen to (i.e. gas/electricity meter top-ups, getting shopping in, feeding pets, prescriptions collected, sat comfortably, etc.)
Manage referrals for patients to access additional services and support
Keep accurate, confidential records of client information and need
Provide and maintain statistical information and case records as required for the efficient running of the service
Collect, monitor and report on service outcomes, ensuring accurate data recording to demonstrate impact and support contract reporting requirements.
Manage safe working practices of lone workers
Provide day-to-day guidance, support and supervision to Home from Hospital staff and volunteers.
Work as part of a team with other Home from Hospital staff and volunteers to ensure the rota provides sufficient cover to operate a responsive service within agreed timescales from referral
Develop and maintain effective working relationships with hospital discharge teams, community health professionals, social care teams and voluntary sector partners.
Liaise with patient carers/family members as needed as part of providing the patient with necessary support
Where required, to attend meetings on behalf of Age UK Manchester, and to promote the work of the the organisation at these meetings
Other such duties as may be required that are consistent with the duties and responsibilities of the post.
