Home carers Care Services is expanding its domiciliary care services across Manchester and Tameside, and we are looking for a dedicated and compassionate Part-Time Care Assessor/Mentor to join our growing team.

As a Care Assessor/Mentor, you will be responsible for assessing referred service users and taking a strength-based, person-centred approach to care planning and risk assessments. Your role will help enable individuals to live as independently as possible in the comfort of their own homes.

About the Role

Key responsibilities include:

Assessing customers' needs and developing personalised support plans, including risk assessments, using a strength-based approach.

Creating flexible care plans that allow customers to make day-to-day changes to their support arrangements.

Using a range of information sources to identify whether the care being provided continues to meet customers' needs.

Providing practical, hands-on assistance when required to complete tasks.

Supporting service delivery as part of contingency planning arrangements.

Keeping up to date with legislation, regulations, and best practice within adult social care, including CQC requirements and contractual changes.

Maintaining effective working relationships with colleagues across the organisation, including Compliance teams, to ensure excellent communication and service delivery.

What We’re Looking For
  • Previous care experience and a minimum Level 3 Diploma in Health and Social Care.
  • Experience of assessment and care planning based on the principles of a strength-based approach.
  • Experience working with health and social care professionals, including District Nursing teams and Allied Health Professionals.
  • Flexibility to work a range of shifts, including alternate 1 in 3 weekends
  • Strong communication and interpersonal skills.
  • A broad understanding of compliance requirements within the social care sector.
  • A valid UK driving licence and access to your own vehicle.
  • A genuine passion for delivering high-quality care.
